This module builds upon the information provided in the Nuts and Bolts of Patient Adherence Module by sharing strategies for effective patient-centered teaching and counseling that improves adherence. Living longer and advancements in medicine and technology require patients to know more and be engaged in their health care. There have been and continue to be major changes in how healthcare is practiced that have raised healthcare providers’ expectations about what patients should know and should be able to do. Use of behavior change and engagement strategies when caring for patients is more essential now than ever. Some of the behavior change strategies that engage patients in their own care and influence adherence which are addressed in this module include:
- Stages of Readiness to Change Behaviors
- Enhancing Self-Efficacy
- Goal Setting and Action Planning
- 5 A's Behavior Change Model: Assess, Advise, Agree, Assist, Arrange
No one approach is superior to other approaches, and in fact, they are often complimentary. The nature and context for a patient seeking services and the person's specific needs and preferences must be considered when deciding on the most appropriate method to effectively engage a patient and promote self-management.

Objectives:
Participants who complete this module will be able to:
- Describe patient engagement, self-management, and the skills needed to become an active self-care manager.
- Describe the 5 A's Behavior Change Model for self-management support.
- Describe the process of helping patients set goals and develop action plans for making changes in behavior.
- Explain the concept of self-efficacy and the four methods to use to enhance a person's confidence level in managing their care.
- Apply strategies to use when a person is in a particular stage of readiness to change.
- Identify the stages of readiness to change behaviors.
